Chronic adrenal infarction presenting as an incidentaloma: Three-year radiological follow-up

Abstract:
This is a descriptive observational study conducted at a University Hospital. We report the case of a 34-year-old woman with a right adrenal mass detected incidentally on computed tomography, presenting atypical radiological characteristics that raised suspicion of malignancy. Hormonal investigation ruled out hypersecretion, and the patient underwent adrenalectomy. Histopathological examination revealed chronic adrenal infarction. Retrospective review of medical records identified a previous episode of severe COVID-19 during pregnancy, and re-evaluation of the computed tomography scan performed at that time allowed us to document the evolution of the adrenal infarction from the acute phase to the formation of a chronic mass over 3 years. The case illustrates an unusual presentation of adrenal infarction as an incidentaloma and highlights the importance of clinical-radiological correlation in the differential diagnosis of adrenal masses. Data collection was performed through review of medical records.
