Objective:
To examine, compared with full-term peers, whether preterm infants are similarly affected by (additive main effect model), or are more vulnerable (diathesis-stress model), or more susceptible to (differential susceptibility model) the home environment on their neuropsychological development.
Methods:
Utilizing a cross-sectional design, the study recruited a convenience sample of 434 infants. Parents completed the Infant Home Nurture Environment Questionnaire, and trained research nurses assessed neuropsychological development using a nationally standardized software derived from the China Developmental Scale for Children. Multiple regression and Region of Significance Analysis were conducted to analyze the interaction patterns between home environment and prematurity status (preterm vs. full-term) on neuropsychological outcomes. Results were also validated using Re-parameterized Regression Model.
Results:
The final analytic sample comprised 68 preterm infants (34 boys, 34 girls; gestational age: 34.7 ± 2.2 weeks; study age: 7.0 ± 2.9 months), and 344 full-term infants (194 boys, 150 girls; study age: 6.5 ± 3.1 months). A significant interaction between home environment and prematurity status was observed for social behavior, conformed to the strong diathesis-stress model, indicating greater vulnerability of preterm infants to low-quality home environment. No significant interactions were observed for gross motor, fine motor, or language development, suggesting additive main effect model and independent contributions.
Conclusions:
High-quality home environment is beneficial regardless of prematurity status, but preterm infants are disproportionately vulnerable to low-quality home environment in social behavior. Interventions to enhance the home environment should be prioritized for preterm infants to improve their neuropsychological outcomes, particularly social behavior.
