  • The primary outcome was successful MTA completion at 18 months, defined by surgery scheduling or sufficient weight loss.
  • Main Results:

    • Overall MTA completion was 64.6% with no significant difference between the app and control groups.
    • However, high app engagement (≥34% usage) led to significantly higher MTA completion (76.4%) compared to low engagement (20.0%) and controls (62.6%).
    • Lower baseline BMI was an independent predictor of successful MTA completion. Quality of life improved, while weight and body composition remained unchanged.

    Conclusions:

    • Mobile health applications can enhance bariatric surgery preoperative assessment completion, contingent on user engagement.
    • Digital health interventions require strategies to foster sustained engagement and support for low-usage patients.
    • Early identification and intervention for patients with low engagement are crucial for maximizing the benefits of digital health tools.
