Introduction:
Introduction: intestinal lymphangiectasia is a rare entity characterized by alteration of intestinal lymphatic drainage, which causes protein-losing enteropathy and fat malabsorption. Dietary treatment is based on the reduction of long-chain triglycerides (LCT) and supplementation with medium-chain triglycerides (MCT), constituting the main therapeutic pillar. Case report: a 70-year-old woman with secondary intestinal lymphangiectasia, chronic diarrhea, hypoproteinemia, and hypogammaglobulinemia. As a relevant history, chemotherapy and radiotherapy for vertebral hemangioendothelioma in 1993. A personalized diet very low in LCT, rich in MCT, with high protein intake and oral supplementation was established. After three years of follow-up, she presented resolution of diarrhea, significant improvement in albuminemia, and progressive weight recovery. Discussion: long-term LCT restriction in secondary intestinal lymphangiectasia is effective in controlling symptoms and improving nutritional status; dietary individualization, MCT supplementation, and continuous follow-up are essential to ensure adherence and prevent nutritional deficiencies.
