Dynamic navigation combined with guide plate-assisted immediate implantation in the posterior maxillary region

Abstract:
Zygomatic implantation addresses insufficient posterior maxillary bone volume for implant restoration, but it presents high technical difficulty and risks. Dynamic real-time navigation can enhance implantation accuracy and safety. The aim of this study is to investigate the clinical application value of dynamic navigation combined with static guide plates for immediate implant placement in patients with severe bone defects in the maxillary posterior region. 31 patients with 48 zygomatic implants were included in this study. The patients were divided into three groups: navigation combined with plate group (17 implants), guide plates group (15 implants), and free-hand group (16 implants). The implant initial stability, the bone-to-implant contact (BIC), implant accuracy, success rate, and restoration time were evaluated. All zygomatic implants were successfully implanted. The navigation combined with template group had higher initial stability and maximum torque values. Additionally, the BIC value and implant accuracy of the navigation combined with plate group were significantly better than those of the guide plate and free-hand implant groups (P < 0.05). In conclusion, the dynamic navigation combined with static guide plates in zygomatic immediate implant placement can achieve accurate implantation, higher primary stability, and reduce technical difficulty of the operation.
