Patient-reported outcome measures (PROMs) show limited correlation with perceived health changes after cervical spine surgery.   • Spearman rank correlation coefficients used to assess the relationship between GRC and changes in PROMs (NDI, VAS, SF-12, PROMIS-PF).

    • Main Results:

    • Three hundred sixteen patients (109 CDRs, 207 ACDFs) were analyzed.
    • Most patients reported improved cervical spine health postoperatively.
    • Statistically significant but moderate correlations were found between preoperative PROM changes and GRC in the CDR cohort; however, postoperative changes showed no significant correlation. In the ACDF cohort, preoperative PROM changes did not correlate significantly with GRC, while some postoperative PROM changes showed moderate correlation with GRC at specific time points.

    Conclusions:

    • Commonly used PROMs in cervical spine surgery showed weak to moderate correlations with patient-perceived health changes.
    • Findings suggest that current PROMs may not adequately capture the health improvements most relevant to patients.
    • Further research is needed to develop or refine PROMs that better reflect patient-centered outcomes in cervical spine surgery.
