SBAR I: Understanding the Concept

Effective communication among healthcare professionals during hand-off reporting is essential to delivering safe and continuous patient care. Common professional interactions include reports to healthcare team members, hand-off, and transfer reports. Nurses routinely report information to other healthcare team members and also urgently contact healthcare providers to report changes in patient status.
Standardized methods of communication have been developed to ensure that information is...

Hemodialysis III: Nursing Management

The nursing management of a patient undergoing hemodialysis includes several critical steps, starting with a thorough assessment before the procedure. Before the Hemodialysis ProcedureFirst, record the patient's vital signs—blood pressure, heart rate, respiratory rate, and temperature—to establish a baseline. This baseline is essential for detecting conditions such as hypotension that could impact the patient's response to dialysis. Document the patient's pre-dialysis weight, as this measurement...

Appendicitis-II: Diagnostic Studies and Management

Diagnosing and managing appendicitis requires a structured and comprehensive approach that spans from initial assessment to postoperative care. Here is an overview of the process:
Diagnosing Appendicitis
It requires a multifaceted approach, starting with a detailed physical examination to pinpoint the location and nature of the pain and identify any associated symptoms. Laboratory tests play a crucial role. A complete Blood Count (CBC) typically reveals leukocytosis (an increased number of...

SBAR II: Application of SBAR

SBAR is an effective communication tool used by healthcare professionals to communicate patient information accurately. SBAR stands for Situation, Background, Assessment, and Recommendation. For a better understanding, an example is given below.

Area of Science:

  • Emergency Medicine
  • Health Informatics
  • Patient Communication

Background:

  • Discharged Emergency Department (ED) patients require clear self-care and return-to-care instructions.
  • Verbal instructions are often forgotten, and traditional written instructions face accessibility barriers.
  • Inefficiencies in delivering and retrieving paper-based instructions hinder optimal patient care.

Purpose of the Study:

  • To assess the feasibility and uptake of a novel website for sending condition-specific ED discharge instructions via text message.
  • To address barriers in delivering timely and accessible post-discharge information to patients.
  • To improve medicolegal compliance and patient understanding of discharge guidance.

Main Methods:

  • A feasibility and uptake study was conducted in Saskatoon, Canada adult EDs.
  • Physicians used a website to send patient-specific discharge instructions via text message.
  • Link access data and physician feedback surveys were analyzed.

Main Results:

  • Over 2 months, 115 instructions were sent; 83% were accessed, with 43% accessed repeatedly.
  • Most links were accessed rapidly: 57% within one minute, 81% within one hour.
  • Physicians rated the system highly for ease of use (87% ≥ 8/10) and efficiency (73% ≥ 7/10), with 93% intending future use.

Conclusions:

  • Despite low initial physician uptake, the system demonstrated high patient engagement with accessed instructions.
  • Positive physician feedback on usability and efficiency suggests potential.
  • The text-based approach shows promise for improving Emergency Department discharge practices.
