Summary

Rituximab plus lenalidomide maintenance did not significantly improve 2-year progression-free survival in elderly patients with relapsed or refractory follicular lymphoma. A potential benefit was observed in patients younger than 70 years.

Background:

  • Follicular lymphoma (FL) is a common indolent non-Hodgkin lymphoma.
  • Maintenance therapy for relapsed or refractory (R/R) elderly FL patients requires further investigation.
  • Standard rituximab-based chemotherapy is a common induction regimen.

Purpose of the Study:

  • To evaluate the efficacy and safety of rituximab plus lenalidomide (R2) maintenance versus rituximab alone (R) in elderly patients with R/R FL.
  • To assess the primary endpoint of 2-year progression-free survival (PFS).

Main Methods:

  • RENOIR was a multicenter, phase III, open-label, randomized trial.
  • 152 elderly patients with R/R FL after 1-2 prior therapies were enrolled.
  • Patients achieving response post-induction were randomized to R or R2 maintenance.

  • The R2 arm had more Grade 3/4 adverse events, primarily neutropenia and gastrointestinal disorders.
  • R2 demonstrated a potentially more favorable benefit/risk profile in patients <70 years.
