Staged resection with vacuum-assisted closure (VAC) did not improve local recurrence rates for superficial soft tissue sarcomas (STS). This approach was associated with higher-risk patients and complex presentations, suggesting limitations in mitigating recurrence risk alone.

Area of Science:

  • Surgical Oncology
  • Oncology
  • Wound Management

Background:

  • Staged resection with vacuum-assisted closure (VAC) is used for soft tissue sarcomas (STS) to aid margin assessment and local control.
  • The effectiveness of VAC in reducing local recurrence (LR) in superficial STS requires further investigation.

Purpose of the Study:

  • To compare local recurrence (LR) rates in superficial soft tissue sarcoma (STS) patients treated with and without temporizing vacuum-assisted closure (VAC) therapy.

Main Methods:

  • A multi-institutional retrospective cohort study.
  • Matching patients by grade and performing multivariate logistic regression.
  • Analysis included 314 superficial STS patients from three tertiary care centers.

  • Patients treated with VAC were more likely to have prior unplanned excision, recurrent disease, myxofibrosarcoma, and positive initial margins.
  • Multivariate analysis indicated that VAC use was independently associated with increased local recurrence (LR) rates (OR = 4.117; p = 0.003).

Conclusions:

  • Staged resection with temporizing VAC was not associated with improved local recurrence (LR) rates in superficial STS.
  • The VAC cohort had higher-risk tumor biology and complex presentations, indicating that VAC alone may not fully mitigate LR risk.
