Methods of Documentation VI: Case Management Model01:15

Methods of Documentation VI: Case Management Model

The case management model is a multidisciplinary approach that involves healthcare professionals from diverse disciplines, such as physicians, nurses, therapists, social workers, and pharmacists, working collaboratively to address the various needs of patients. Each healthcare professional brings unique expertise and perspectives, contributing to a more comprehensive understanding of the patient's condition and tailoring treatment plans accordingly.
For example, a patient with a chronic illness...

Methods of Documentation II: POMR

The Problem-Oriented Medical Record (POMR) revolutionized medical record-keeping by introducing a systematic approach focusing on the patient's problems rather than merely listing symptoms. Dr. Lawrence Weed's introduction of this method in the 1960s marked a significant advancement in medical documentation. The POMR framework consists of four key components: the database, problem list, plan of care, and progress notes.
Analgesia and Pain Management01:25

Analgesia and Pain Management

Pain is critical to various clinical pathologies, provoking an urgent need for effective management. Pain, whether acute or chronic, is a complex neurochemical process. Its alleviation depends on the type, with nonopioid analgesics effective for mild to moderate pain, such as musculoskeletal or inflammatory pain, while neuropathic pain responds best to anticonvulsants, tricyclic antidepressants, or serotonin/norepinephrine reuptake inhibitors. For severe acute or chronic pain, opioids may be...

Peripheral Artery Disease III: Interprofessional Care

Peripheral Artery Disease (PAD) is characterized by narrowed arteries that diminish blood flow to the extremities. Effective management of PAD requires an interprofessional approach involving various healthcare professionals. The critical aspects of interprofessional care for PAD patients focus on risk factor modification, drug therapy, exercise therapy, nutrition therapy, critical limb ischemia care, and interventional radiology and surgical procedures. The primary treatment goal for PAD...

Chronic Bowel Disorders: Introduction

Chronic bowel diseases are a group of long-term conditions affecting the digestive tract, characterized by inflammation and damage to the gut lining. These conditions primarily include irritable bowel syndrome and inflammatory bowel disease.
Irritable Bowel Syndrome (IBS) is a common disorder affecting the gastrointestinal tract. The distinctive feature is recurrent abdominal pain associated with altered bowel movements, manifesting as constipation, diarrhea, or fluctuating between both. The...

Area of Science:

  • Pain Medicine
  • Multimorbidity Research
  • Nociplastic Pain Mechanisms

Background:

  • Chronic overlapping pain conditions (COPCs) are distinct disorders that frequently co-occur.
  • Their coexistence leads to severe multimorbidity, impacting pain, psychological well-being, function, and quality of life.
  • COPCs are often under-recognized, underdiagnosed, and undertreated.

Purpose of the Study:

  • To review the comorbid relationships and multimorbidity burden of ten recognized COPCs.
  • To examine potentially overlapping nociplastic mechanisms underlying COPCs.
  • To propose integrated management strategies for COPCs.

Main Methods:

  • Structured literature searches and citation tracking.
  • Narrative synthesis of findings.
  • Analysis of comorbid relationships and mechanistic overlaps.

Main Results:

  • Multiple COPCs are associated with disproportionately greater pain, psychological distress, functional limitations, and healthcare utilization.
  • Barriers to care include limited clinical awareness, under-recognition, poor mechanistic understanding, and fragmented care.
  • Overlapping nociplastic mechanisms may contribute to the co-occurrence of COPCs.

Conclusions:

  • A multimorbidity-based perspective is crucial for managing COPCs, moving beyond single-disease approaches.
  • Integrated strategies focusing on prevention, screening, mechanism-informed assessment, and coordinated care are needed.
  • Improved recognition and management of COPCs can reduce patient burden and healthcare costs.
