Background:

  • Behçet's syndrome (BS) can cause retinal vasculitis, affecting vision.
  • Optical coherence tomography angiography (OCTA) and microperimetry assess retinal microvasculature and function.
  • Integrated structure-function analysis in inactive BS retinal vasculitis is limited.

Purpose of the Study:

  • Evaluate age- and sex-related differences in inactive BS retinal vasculitis.
  • Analyze structure-function associations using OCT and OCTA.
  • Assess 12-month changes in retinal biomarkers during clinical remission.

Main Methods:

  • Observational study of 14 patients (23 eyes) with inactive BS retinal vasculitis.
  • 12-month follow-up with OCT, OCTA, and microperimetry to assess retinal thickness, foveal avascular zone (FAZ) area, vascular density (VD), and macular sensitivity (MS).
  • Exploratory analyses by sex, age, and OCTA findings; statistical analysis using generalized estimating equations and Spearman's correlation.

Main Results:

  • Male sex, younger age (<45 years), and specific OCTA findings correlated with more significant structural and vascular changes.
  • Vascular density (VD) showed strong correlations with foveal avascular zone (FAZ) area and retinal thickness.
  • Superficial vascular plexus (SVP) VD correlated directly with central macular sensitivity (MS).

Conclusions:

  • Younger males with BS retinal vasculitis may experience greater macular involvement.
  • Qualitative OCTA findings, especially in the superficial and intermediate plexuses, are linked to structural and functional alterations.
  • Preliminary findings require validation in larger, controlled studies due to the exploratory nature and small sample size.
