    Summary

    PlastiPore prostheses with interposed cartilage improved hearing outcomes in ossicular reconstruction. This technique achieved better results than using ossicles or cartilage alone for total and partial ossicular replacement prostheses.

    Background:

    • Ossicular reconstruction aims to restore hearing by replacing damaged or missing middle ear bones.
    • Traditional methods using autologous ossicles or cartilage have limitations in achieving consistent hearing outcomes.
    • Prosthetic materials offer an alternative, but extrusion and suboptimal hearing results remain challenges.

    Purpose of the Study:

    • To evaluate the efficacy of PlastiPoreTM prostheses for ossicular reconstruction.
    • To assess the impact of interposing cartilage between the prosthesis and tympanic membrane graft on hearing outcomes.
    • To compare the results of PlastiPore prostheses with historical data using other reconstruction methods.

    Main Methods:

    • Retrospective review of 106 cases utilizing PlastiPoreTM prostheses for ossicular reconstruction.
    • In all cases, a piece of cartilage was placed between the prosthesis and the tympanic membrane graft.
    • Audiometric outcomes were assessed by measuring the air-bone gap closure relative to bone conduction levels.

    Main Results:

    • Total ossicular replacement prostheses (TORPs) achieved closure to within 10 dB in 55% of cases and within 20 dB in 85% of cases.
    • Partial ossicular replacement prostheses (PORPs) achieved closure to within 10 dB in 64% of cases.
    • The use of interposed cartilage successfully prevented prosthesis extrusion in all reviewed cases.

    Conclusions:

    • PlastiPoreTM prostheses, when combined with interposed cartilage, provide effective hearing restoration in ossicular reconstruction.
    • This technique demonstrates superior audiological results compared to previous methods using only ossicles or cartilage.
    • The interposition of cartilage is a valuable adjunct for preventing prosthesis extrusion and improving outcomes.
