BMP-7 links delayed memory impairment to severity of anhedonia in depression: A mediation model

Background:
Anhedonia, a core symptom of major depressive disorder (MDD), is closely associated with cognitive deficits and elevated suicide risk. This study aimed to investigate the relationship between inflammatory markers and anhedonia.
Methods:
A total of 74 patients with MDD and 70 healthy controls were recruited. Patients were stratified into anhedonic MDD (A_MDD, N = 30) and non-anhedonic MDD (nA_MDD, N = 44) groups based on the Dimensional Anhedonia Rating Scale (DARS). The Hamilton Rating Scale for Depression-24 (HAMD-24), the Hamilton Anxiety Rating Scale-14, and the Repeatable Battery for the Assessment of Neuropsychological Status were used to evaluate clinical symptoms and cognitive function. Serum levels of BMP-7, BMP-9, BMP-10, IL-7, and IL-10 were measured.
Results:
Compared to nA_MDD, A_MDD exhibited poorer performance in language and delayed memory (both q < 0.001), with elevated BMP-7 and decreased IL-10 (q < 0.001 and q = 0.019, respectively). BMP-7 levels were negatively correlated with DARS (q < 0.001), language (q = 0.007), and delayed memory (q = 0.007), and positively correlated with HAMD (q = 0.014). IL-10 levels were positively correlated with DARS (q = 0.045), language (q = 0.020), and delayed memory (q = 0.020). Hierarchical regression analysis identified BMP-7 (P = 0.026) and delayed memory (P < 0.001) as independent predictors of anhedonia. Exploratory mediation analysis suggested that BMP-7 may partially mediate the relationship between delayed memory and anhedonia.
Conclusion:
Anhedonia has unique inflammatory immune characteristics. BMP-7 may constitute a key inflammatory pathway linking delayed memory and anhedonia.
