  • Postoperative pain was assessed using a visual analog scale (VAS) at 24h, 48h, 72h, and 7 days.
  • Main Results:

    • All rotary systems showed a statistically significant reduction in VAS scores over time (p < 0.001).
    • The TruNatomy group exhibited the lowest mean pain scores throughout the study period.
    • Significantly lower pain scores were observed in the TruNatomy group compared to GenEndo and Jizai at all time points (p = 0.001).

    Conclusions:

    • All tested rotary systems effectively reduced postoperative pain over time.
    • The TruNatomy system was associated with significantly lower postoperative pain compared to GenEndo and Jizai.
    • The flexible and minimally invasive design of TruNatomy may contribute to reduced periapical irritation and enhanced patient comfort.
