Abstract:
Using a clinical and pragmatic approach, the commentary evaluates Fisher's (see record 2026-72441-001) proposal to replace traditional fixed symptom counts for internalizing disorders with "minimally sufficient" symptom constellations that predict clinically meaningful outcomes such as distress and functional impairment. Several points are raised. First, there is a concern of circularity, because the same Diagnostic and Statistical Manual of Mental Disorders (DSM)-mapped symptom items are used both to define diagnoses and to derive new constellations. It is argued that the most defensible inferences arise when distress and impairment, rather than DSM labels, are used as external anchors. Second, the approach proposed by Fisher provides a conceptual advance in identifying a small library of irreducible, nonexchangeable symptom sets that reduce combinatorial heterogeneity and link directly to outcome probabilities. These prototypes are simultaneously agnostic to DSM categories and clinically actionable. Third, recurring elements (e.g., uncontrollable worry, posttraumatic stress disorder triads) can serve as hypothesis-generating building blocks for mechanistic and computational studies. At the same time, there are several limitations, which are due to base rates; dichotomized outcomes; design choices (e.g., probability thresholds, structural constraints); and the cross-sectional, associational design. Finally, the commentary outlines a translational road map in which minimal sets are embedded in measurement-based care and collaborative-care workflows as triage tools and used to enrich mechanistic research that combines longitudinal phenotyping, computational modeling, and perturbational designs. Taken together, the commentary positions Fisher's framework as a pragmatic bridge between nosology, clinical decision support, and mechanistic psychiatry rather than a fully realized new taxonomy.
