Summary

Paclitaxel (PTX) and nanoparticle paclitaxel (nab-PTX) offer modest efficacy for relapsed small cell lung cancer (SCLC) with a generally favorable safety profile. Caution is advised for patients with interstitial lung disease (ILD) due to exacerbation risks.

Background:

  • Limited evidence-based treatment options exist for relapsed small cell lung cancer (SCLC).
  • Solvent-based paclitaxel (PTX) and albumin-bound nanoparticle paclitaxel (nab-PTX) are frequently used but lack comprehensive efficacy quantification.

Purpose of the Study:

  • To systematically review and meta-analyze the efficacy and safety of paclitaxel (PTX) and nanoparticle paclitaxel (nab-PTX) monotherapy in relapsed SCLC.
  • To compare outcomes between Asian and Euro-American cohorts and between PTX and nab-PTX treatments.

Main Methods:

  • A systematic review and single-arm meta-analysis of 15 studies (631 patients) reporting PTX or nab-PTX monotherapy in relapsed SCLC.
  • Searches conducted in PubMed, Web of Science, Cochrane Library, and EMBASE.
  • Primary outcomes included 3-month progression-free survival (PFS), 6-month overall survival (OS), objective response rate (ORR), and disease control rate (DCR).

Main Results:

  • Pooled 3-month PFS was 39%, 6-month OS was 46%, ORR was 16%, and DCR was 51%.
  • Grade ≥3 neutropenia occurred in 18% and peripheral neuropathy in 2% of patients.
  • Asian cohorts showed higher neutropenia rates (26% vs. 5%). Efficacy and safety were similar between PTX and nab-PTX.

Conclusions:

  • Paclitaxel-based regimens demonstrate modest, clinically meaningful activity and a generally favorable safety profile in relapsed SCLC, serving as a pragmatic salvage option.
  • Use in patients with interstitial lung disease (ILD) requires caution due to a 17% risk of ILD exacerbation.
