Background:

  • Patient-Reported Outcomes Measurement Information System (PROMIS) is underutilized in foot and ankle fracture recovery.
  • The added value of condition-specific Patient-Reported Outcomes (PROs) over PROMIS in this cohort is unclear.

Purpose of the Study:

  • Evaluate PROMIS Computerized Adaptive Test (CAT) measurement properties.
  • Compare PROMIS CATs with region-specific instruments (FAOS, LEFS, SMFA).
  • Identify factors associated with poorer outcomes in foot and ankle fracture patients.

Main Methods:

  • Prospective, cross-sectional study of 73 patients with foot or ankle fractures.
  • PROMIS Mobility (MOB) and Pain Interference (PI) CATs were compared with FAOS, LEFS, and SMFA.
  • Assessed convergent validity, reliability, efficiency, and floor/ceiling effects; used multivariable linear regression.

Main Results:

  • PROMIS-MOB and PROMIS-PI demonstrated high correlations with lower extremity and pain-focused domains, respectively.
  • PROMIS CATs showed excellent reliability, required fewer items, and less completion time than legacy instruments.
  • Greater pain intensity and depressive symptoms were linked to worse mobility and higher pain interference.

Conclusions:

  • PROMIS CATs provide comparable clinical insights to condition- and region-specific instruments for foot and ankle fractures.
  • PROMIS CATs substantially reduce patient burden compared to traditional instruments.
  • Psychological factors, particularly depressive symptoms, significantly influence recovery outcomes.
