Omega-3 Index: an emerging biomarker in cardiovascular prevention

Introduction:
Long-chain omega-3 fatty acids (EPA and DHA) have significant beneficial effects on cardiovascular health. However, there is considerable controversy regarding results of various clinical trials, that may be attributable to the fact that most studies are based on estimated intake, without determining the actual incorporation of omega-3 fatty acids into tissue membranes. The Omega-3 Index is the sum of percentages of EPA and DHA present in erythrocyte membranes, reflecting the nutritional status of these fatty acids in the heart and other organs. This paper reviews the evidence for the Omega-3 Index as a biomarker of cardiometabolic risk. Omega-3 Index values correlate better with cardioprotective effects than dietary questionnaires. Levels below 4 % are associated with a very high risk of cardiovascular events and sudden death, whilst values above 8 % are considered indicative of high cardiovascular protection. Thanks to its ability to predict all-cause mortality and the incidence of cardiometabolic diseases, it has been considered a new biomarker of cardiovascular risk, not only in research but also in clinical practice.
