Background:

  • Open pronation-external rotation (PER) ankle fracture-dislocations represent severe injuries demanding prompt surgical intervention.
  • Limited evidence exists regarding the efficacy and safety of single-stage management for these complex fractures.

Purpose of the Study:

  • To assess the clinical outcomes of single-stage surgical treatment for open PER ankle fracture-dislocations.
  • To evaluate functional recovery and complication rates in patients without neurovascular compromise.

Main Methods:

  • A retrospective case series of 58 patients with open PER ankle fracture-dislocations treated between December 2013 and July 2024.
  • All patients underwent emergency debridement, open reduction, and internal fixation in a single stage.
  • Outcomes were assessed using the American Orthopaedic Foot & Ankle Society (AOFAS) Ankle-Hindfoot Scale, with radiographic evaluation for complications.
