Introduction:
The aim of current study was to assess Aditi-Nuzhat lymph node prediction score (ANLPS) system, Martínez-Gimeno scoring system (MGSS), and Budding & Depth of Invasion (BD) model for their association with nodal metastasis and overall survival in early-stage OSCC.
Methods And Materials:
Archived paraffin-embedded OSCC tissue blocks from 507 cT1-2N0M0 patients who underwent primary tumor resection and neck dissections (2017-2021) were assessed. Chi-square and Spearman's rank correlation tests evaluated associations with LNM, while binary logistic regression estimated predictive accuracy. Kaplan-Meier curves represented survival functions, applying a significance level of 5%.
Results:
Spearman's coefficients showed a moderate degree of agreement among the scoring systems. A significant association (p < 0.001) was found between LNM and overall survival. Pearson chi-square statistic between the scoring system and LNM yielded the highest value for the MGSS (180.595). An overall accuracy of 82.1% was observed in MGSS using binary logistic regression. The 3-year survival probability was 80%.
Discussion:
MGSS performed comparably to ANLPS and the BD model in predicting LNM, with a higher degree of statistical significance. The results underscore the potential of MGSS in assisting clinicians with decision-making regarding elective neck dissection (END) for patients with early-stage OSCC, providing critical prognostic insights for tailored treatment strategies.
