Insights

Concomitant use of vascular endothelial growth factor receptor tyrosine kinase inhibitors (VEGFR-TKIs) with bone resorption inhibitors (BRIs) did not independently increase medication-related osteonecrosis of the jaw (MRONJ) risk. Denosumab and smoking were key predictors of MRONJ.

Area of Science:

  • Oncology
  • Pharmacology
  • Oral Surgery

Background:

  • Medication-related osteonecrosis of the jaw (MRONJ) is a potential risk with combined use of vascular endothelial growth factor receptor tyrosine kinase inhibitors (VEGFR-TKIs) and bone resorption inhibitors (BRIs).
  • The independent contributions of VEGFR-TKIs, BRI type, and patient factors to MRONJ risk are not fully understood.

Purpose of the Study:

  • To investigate the association between concomitant VEGFR-TKI and BRI use and the risk of developing MRONJ.
  • To identify independent predictors of MRONJ and skeletal-related events (SREs) in patients receiving BRIs.

Main Methods:

  • Retrospective review of patients treated with BRIs, with or without concomitant VEGFR-TKIs.
  • Primary endpoint: MRONJ-free survival. Secondary endpoints: MRONJ incidence and SREs.

Main Results:

  • Concomitant VEGFR-TKI use was not an independent predictor of MRONJ-free survival after adjusting for covariates.
  • Denosumab use and active smoking were identified as independent predictors of MRONJ.
  • VEGFR-TKIs accelerated MRONJ onset in bisphosphonate (BP)-treated patients but not in denosumab-treated patients.
  • MRONJ incidence did not significantly differ between groups, but BRI exposure was shorter in the combination group.

Conclusions:

  • MRONJ risk is influenced by BRI type and individual patient characteristics.
  • Denosumab use is associated with a higher MRONJ risk, irrespective of VEGFR-TKI use.
  • VEGFR-TKIs may hasten MRONJ onset in patients on bisphosphonates, necessitating careful risk-benefit assessment for treatment choices.

Treatment for Pulmonary Arterial Hypertension: Endothelin Receptor Antagonists

Endothelins (ETs) are potent vasoactive peptides critical in the human body's various physiological and pathological processes. One of the most promising therapeutic strategies for treating pulmonary arterial hypertension (PAH) involves counteracting the effects of these endothelins using a class of drugs known as endothelin receptor antagonists.
ETs are synthesized through a complex sequence of enzymatic steps, primarily involving an enzyme referred to as endothelin-converting enzyme (ECE). Of...

Treatment for Pulmonary Arterial Hypertension: Receptor Tyrosine Kinase Inhibitors and Calcium Channel Blockers

Receptor tyrosine kinase inhibitors (TKIs) and calcium channel blockers (CCBs) are two critical categories of drugs employed in the treatment of pulmonary artery hypertension (PAH). PAH is a disease that causes high blood pressure in the pulmonary arteries, resulting in chest pain, fatigue, and shortness of breath.
TKIs, such as imatinib (Gleevec), are particularly effective in tackling the growth and mitogenic factors that become upregulated in PAH patients. These factors contribute to the...

Antihypertensive Drugs: Direct Renin Inhibitors

The renin-angiotensin-aldosterone system (RAAS) is an intricate physiological pathway involving numerous enzymes and hormones, including renin, angiotensin-converting enzyme (ACE), angiotensin I and II, and aldosterone. Imbalances within this system increase the production of angiotensin II and aldosterone. Increased angiotensin II levels promote vasoconstriction and blood pressure elevation. Concurrently, higher aldosterone levels stimulate sodium and water reabsorption in the kidneys,...

mTOR Signaling and Cancer Progression

The mammalian target of rapamycin or mTOR protein was discovered in 1994 due to its direct interaction with rapamycin. The protein gets its name from a yeast homolog called TOR. The mTOR protein complex in mammalian cells plays a major role in balancing anabolic processes such as the synthesis of proteins, lipids, and nucleotides and catabolic processes, such as autophagy in response to environmental cues, such as availability of nutrients and growth factors.
The mTOR pathway or the...

Heart Failure Drugs: Inhibitors of Renin-Angiotensin System

The activation of the sympathetic nervous system and the renin-angiotensin-aldosterone system (RAAS) contributes to cardiac remodeling, and inhibiting the RAAS is a pharmacological target in heart failure management. As a result, neurohumoral modulation is a crucial treatment principle for managing heart failure. This approach involves using medications like ACE inhibitors (ACEIs), angiotensin receptor blockers (ARBs), β-blockers, mineralocorticoid receptor antagonists (MRAs), and neutral...
