Transitional talalgia after medial gastrocnemius lengthening using the Strayer technique in chronic plantar fasciitis

Background:
Chronic plantar fasciitis is frequently associated with isolated gastrocnemius contracture. When conservative treatment fails, medial gastrocnemius recession is an established surgical option. We identified a postoperative pain pattern distinct from preoperative symptoms, defined as transitional talalgia. This study aimed to evaluate postoperative heel pain evolution, describe its characteristics, and assess patient-reported outcomes following Strayer medial gastrocnemius recession.
Methods:
A retrospective observational study included patients who underwent Strayer medial gastrocnemius recession for chronic plantar fasciitis between 2014 and 2024. Demographic variables, body mass index, postoperative heel pain patterns, complications, VAS pain scores, and satisfaction were analysed.
Results:
Thirty-nine patients were included. Mean age was 51.8 years and mean follow-up was 79.8 months. Transitional talalgia was observed in 30.8% of patients. Overall satisfaction was 84.7%. No significant associations were found between body mass index, complications, or pain evolution.
Conclusions:
Transitional talalgia appears frequent, self-limiting, and not necessarily indicative of treatment failure.
