Negative-Pressure Lung Re-expansion After One-Lung Ventilation for Pulmonary Resection: A Retrospective Study

Objectives:
To evaluate the safety of negative-pressure re-expansion of the collapsed lung following one-lung ventilation for lung resection.
Design:
Retrospective observational study.
Setting:
Single tertiary care hospital.
Participants:
Patients who underwent one-lung ventilation for minimally invasive lung resection.
Interventions:
Negative- versus positive-pressure lung re-expansion following one-lung ventilation.
Measurements And Main Results:
There were 242 patients in the negative-pressure re-expansion group and 152 patients in the conventional positive-pressure re-expansion group. No statistically significant differences were observed between groups in early postoperative atelectasis, lung contusion, or pulmonary edema. In addition, no differences were observed in overall complications and respiratory complications at 30 days.
Conclusions:
Negative-pressure lung re-expansion was not associated with increased adverse events compared with conventional positive-pressure re-expansion after one-lung ventilation for minimally invasive lung resection.
