Objective:
To examine the effects of a structured Roy Adaptation Model (RAM)-based nursing intervention on psychosocial adjustment and psychological resilience in women newly diagnosed with breast cancer.
Methods:
This assessor-blinded randomized controlled trial was conducted in a university hospital medical oncology outpatient clinic. Fifty-one women were randomized to an intervention group (n = 25) or a control group (n = 26); 44 participants completed the study and were included in the per-protocol analysis (intervention: n = 21; control: n = 23). The intervention group received a six-session, weekly RAM-based nursing program (standardized and individualized components) in addition to standard psycho-oncology care, while the control group received standard psycho-oncology care alone. Outcomes were measured at baseline (T1), postintervention (T2), and 3-month follow-up (T3) using the Psychosocial Adjustment to Illness Scale-Self Report (PAIS-SR) and the Psychological Resilience Scale (PRS).
Results:
Repeated measures ANOVA showed significant group × time interaction effects for both outcomes. Compared with the control group, the intervention group demonstrated greater improvements in psychosocial adjustment (lower PAIS-SR scores) and psychological resilience (higher PRS scores) over time (both P < .001).
Conclusion:
An early, structured RAM-based nursing intervention delivered alongside standard psycho-oncology care was associated with improved psychosocial adjustment and higher psychological resilience during the early postdiagnosis period.
Implications For Nursing Practice:
Oncology nurses may incorporate RAM-guided assessment and tailored interventions early in the cancer trajectory to address multidimensional adaptation needs and support adaptive coping during treatment.
Trial Registration:
ClinicalTrials.gov (NCT06586944).
