Editorial Commentary: The Lateral Femoral Condyle Angle: Adding Another Piece to the Complex Puzzle of Anterior

Predicting risk of anterior cruciate ligament (ACL) injury and ACL graft rupture remains an elusive challenge, with a substantial and emerging body of literature attempting to elucidate the clinical and anatomic factors that influence risk. In particular, anatomic risk factors have gained interest as advancements in imaging technology have conferred an enhanced ability to identify and quantify novel variables with differential magnitudes of clinical importance. The lateral femoral condyle angle represents one such novel metric that accounts for the relative flexion or extension of the lateral femoral condyle relative to the femoral axis and withholds a feasible biomechanical basis for injury risk. Despite reasonable predictive performance on this dataset, important challenges still remain in attempting to mitigate ACL injury risk. ACL injury risk is inherently multifactorial, arising from the interaction of biomechanical, neuromuscular, anatomical, and environmental influences. The lateral femoral condyle angle described provides incremental insight into this complex system, but should not be interpreted in isolation. Rather, it represents one component within a broader constellation of risk factors, where meaningful prediction and prevention will depend on integrated, multivariable approaches rather than reliance on any single parameter.
