Infection prevention practices in Thai hospitals: 15-year national trends and the role of institutional support

Background:
Since 2010, national surveys have tracked infection prevention practices in Thailand. Using 2025 data, we review 15-year trends in practice adoption, institutional support, and workforce well-being.
Methods:
From January to June 2025, 107 Thai hospitals were surveyed regarding urinary tract (CAUTI), bloodstream (CLABSI), and ventilator-associated (VAE) infection practices, alongside safety culture and burnout. Trends across four surveys (2010-2025) were evaluated using Cochran-Armitage tests.
Results:
The response rate for the 2025 survey was 100%. "Very good/excellent" institutional support remained unchanged from 2021 at 31%. Over 15 years, key practices improved significantly (P < .001): CAUTI- catheter reminder systems increased from 33.3% in 2010 to 74.8% in 2025; CLABSI- chlorhexidine antisepsis rose from 37.7% in 2010 to 96.3% in 2025; and VAE- sedation vacation increased from 36.8% in 2014 to 72.0% in 2025. Semi-recumbent positioning remained high (92.5%, P = .024). Stronger support correlated with adoption. Workforce burnout increased from 29% in 2021 to 43% in 2025.
Conclusions:
While specific infection prevention practices improved, institutional support remains limited despite its strong link to practice adoption. Renewed national strategies are urgently needed to strengthen Thailand's infection prevention infrastructure and support professional well-being.
