Microbiological profile of hemodialysis water treatment systems from a high-volume center

Purpose:
To document the microbiological isolates in treated water samples obtained from a hemodialysis center and investigate potential correlations between these isolates and the microorganisms identified in blood cultures from patients diagnosed with catheter-related bloodstream infections (CRBSIs) during the period.
Methods And Results:
From May 2019 to December 2023, monthly samples were collected from eight designated points in the water treatment system.
Results:
Positive cultures accounted for 10.14% of samples, with 52.87% in the pre-stage and 49.1% in the post-filt stage. Predominant microorganisms included Sphingopyxis spp., Pseudomonas spp., and Brevundimonas aurantiaca. Microbiological growths were not observed in sampling point after ultraviolet radiation (UV). One CRBSI due to Pseudomonas was reported, with no temporal link between isolation in blood and water.
Conclusions:
Despite low pathogenicity in most cases, microbiological control in water needs strict monitoring and effective water treatment systems. Enhanced procedures such as UV light filtration should be considered to minimize risks in patients.
