EGFR mutation status does not impact recurrence or disease-free survival in resected non-small cell lung cancer (NSCLC). However, adjuvant EGFR-tyrosine kinase inhibitors (TKIs) significantly improve survival, supporting their use post-surgery.

  • Calculated pooled recurrence rates, risk ratios (RR), and hazard ratios (HR) using random-effects models.
  • Main Results:

    • Analysis of 20 studies (12,123 patients) showed no significant difference in recurrence (RR 1.04) or DFS (HR 1.07) between EGFR-mutant and wild-type NSCLC.
    • Recurrence patterns, including local, distant, and central nervous system relapse, were comparable.
    • Adjuvant EGFR-tyrosine kinase inhibitors (TKIs) significantly improved DFS in EGFR-mutant NSCLC (HR 0.45), whereas chemotherapy did not (HR 1.05).

    Conclusions:

    • EGFR mutation status is not a prognostic factor for recurrence or DFS in resected NSCLC.
    • Adjuvant EGFR-TKIs demonstrate significant DFS benefit in EGFR-mutant NSCLC, suggesting their role in postoperative treatment.
    • Chemotherapy does not appear to provide a DFS benefit in this context.
