This is a rare case of rectovesical fistula following repair of esophageal rupture in a critically ill patient. Sepsis and prior prostatectomy may have contributed. CT cystography confirmed the diagnosis, and initial fecal diversion controlled inflammation. Longitudinal follow-up and staged management are essential to guide definitive treatment and improve outcomes.
