Summary

Ex vivo normothermic perfusion (EVNP) quality scores can predict viability in discarded human kidneys after 6 hours. This assessment may help reduce organ discard by identifying suitable marginal grafts.

Background:

  • Current scoring systems for ex vivo normothermic perfusion (EVNP) are validated for short durations.
  • This study evaluates the EVNP quality assessment score for a 6-hour period to predict viability in discarded human kidneys.

Purpose of the Study:

  • To investigate the utility of a 6-hour EVNP quality assessment score in predicting the viability of discarded human kidneys.
  • To determine if EVNP can help reduce organ discard by identifying suitable marginal grafts.

Main Methods:

  • Twenty human kidneys declined for transplantation underwent 6-hour EVNP.
  • Functional parameters (renal blood flow, resistance index, urine output), perfusate biochemistry, and histology were assessed.
  • Kidneys were stratified into groups based on EVNP score (A: 1-2, B: 3, C: 4-5).

Main Results:

  • Group A (higher EVNP score) showed significantly better renal blood flow, lower resistance index, and greater urine output compared to groups B and C.
  • Favorable perfusate lactate levels were observed in Group A.
  • Histological scores did not correlate with EVNP score groups, indicating EVNP may identify viable kidneys despite suboptimal histology.

Conclusions:

  • The 6-hour EVNP score shows potential in predicting kidney viability for transplantation.
  • EVNP assessment may help reduce the underutilization of marginal kidney grafts, even those with suboptimal histology.
