Sciatic nerve (SN) division shows high variability, challenging surgical procedures. A new sciatic nerve division index (SNDI) offers precise sub-classification for improved anatomical localization and procedural safety.

Background:

Sciatic nerve (SN) division exhibits significant anatomical variability.
Existing classification models are inadequate for precise sub-classification of SN bifurcation.
This variability poses challenges in orthopaedic, neurosurgical, and anaesthetic procedures.

Purpose of the Study:

Introduce a novel sciatic nerve division index (SNDI).
Enable precise sub-classification of SN bifurcation levels.
Enhance anatomical localization for improved procedural safety.

Main Methods:

Dissection of 58 lower limbs from 29 adult cadavers.
Tracing SN bifurcation beyond the piriformis muscle (Beaton and Anson Type A).
Subtyping bifurcation levels (AI, AII, AIII) using the SNDI morphometric tool.

Main Results:

Extra-pelvic (Type A) bifurcation observed in 72.41% of limbs.
Mid-thigh (Type AII) bifurcation was most frequent (44.8%).
Proximal (Type AI) and distal (Type AIII) bifurcations occurred in 29.3% and 25.9%, respectively; non-Type A variants comprised 27.58%.

Conclusions:

High variability in SN division necessitates improved classification models.
The SNDI system enhances consistency and anatomical localization of SN bifurcation.
This leads to improved procedural safety and informs future research and education.
