Area of Science:

  • Hematology
  • Transplant Immunology
  • Apheresis Technology

Background:

  • Accurate prediction of final CD34+ cell yield is crucial for optimizing allogeneic peripheral blood stem cell (PBSC) harvests.
  • Reducing processed blood volume (PBV) during apheresis can decrease donor burden while ensuring adequate stem cell collection.
  • Current prediction methods may lack the precision needed for efficient PBV management.

Purpose of the Study:

  • To compare the accuracy of CD34+ cell yield predictions using pre-procedure versus interim apheresis product measurements.
  • To evaluate the feasibility of optimizing PBV during apheresis based on interim CD34+ cell concentration.
  • To identify factors associated with significant decreases in CD34+ cell concentration during apheresis.

Main Methods:

  • Retrospective analysis of 86 apheresis procedures in 81 healthy donors.
  • Comparison of prediction errors between pre-procedure peripheral blood CD34+ cell concentration and interim collection-product CD34+ cell concentration (at ~50 mL/kg PBV).
  • Evaluation of CD34 percent change (CD34% change) and associated factors, including pre-procedure platelet count.

Main Results:

  • Interim CD34+ cell-based prediction demonstrated significantly smaller absolute prediction error compared to pre-procedure prediction (p=0.034).
  • Median CD34% change was -3.6%, with 27.9% of procedures showing a decrease of 10% or more.
  • Higher pre-procedure platelet count was associated with a greater decrease in CD34+ cell concentration during apheresis.

Conclusions:

  • Interim CD34+ cell concentration measurement during apheresis provides more accurate yield estimations.
  • This improved prediction accuracy may allow for optimized PBV management, potentially reducing apheresis duration and donor exposure.
  • The findings support the use of real-time monitoring during apheresis to enhance stem cell collection efficiency and donor safety.
