Microsaccade dynamics, or tiny eye movements, improve pulmonary nodule detection in chest radiography. This research introduces a novel physiological biomarker for assessing radiologist expertise in interpreting chest X-rays.

Area of Science:

  • Ophthalmology
  • Radiology
  • Cognitive Neuroscience

Background:

  • Perceptual misses are a key cause of diagnostic errors in radiology.
  • Currently, no physiological biomarker exists to measure expertise in chest radiograph interpretation.

Purpose of the Study:

  • To investigate if microsaccade dynamics improve pulmonary nodule detection.
  • To determine if microsaccade dynamics can differentiate between levels of reader expertise in chest radiography.

Main Methods:

  • An experimental eye-tracking study involving 26 adults (naive observers, residents, attending radiologists).
  • Readers searched for nodules in 148 chest radiographs.
  • Microsaccade dynamics were analyzed in relation to nodule regions and trial accuracy, with receiver operating characteristic analyses used to differentiate reader accuracy levels.

Main Results:

  • On correct trials, microsaccades per fixation were higher when viewing nodules compared to other regions, irrespective of expertise level.
  • This microsaccade pattern was absent on incorrect trials, linking it to successful detection.
  • Higher-accuracy readers exhibited a greater frequency of fixations with microsaccades in the target region during correct trials.
  • A microsaccade-based oculomotor signature successfully differentiated lower- from higher-accuracy readers.

Conclusions:

  • Microsaccade dynamics enhance the detection of pulmonary nodules in chest radiography.
  • This study presents the first microsaccade-based physiological biomarker for assessing expertise in chest radiograph interpretation.
