First Report of Circular Rainbow Glare After Femtosecond Laser-Assisted LASIK

Purpose:
To describe the first documented case of circular rainbow glare following laser in situ keratomileusis (LASIK) with a spiral flap creation pattern.
Methods:
Case report.
Results:
A 62-year-old woman presented with visual discomfort after undergoing monovision LASIK at an external center. Incorrect eye dominance selection resulted in unintended myopization of the dominant eye. The patient reported difficulty with distance vision and described multi-colored rings surrounding the point light sources in dim environments. LASIK enhancement was planned by relifting the stromal flap to achieve emmetropia in the dominant eye. Intraoperative findings revealed a concentric pattern of femtosecond laser impacts on the posterior surface of the flap and the stromal bed. The patient reported complete resolution of rainbow glare symptoms in the treated eye and an improvement in uncorrected distance visual acuity.
Conclusions:
This case represents the first reported instance of circular rainbow glare, supporting a diffractive etiology related to the periodic arrangement of femtosecond laser spot placement.
