Paget-Schroetter Syndrome with a Pulmonary Embolism in a High School Volleyball Coach

Paget-Schroetter syndrome is a rare upper-extremity deep vein thrombosis caused by repetitive mechanical compression of the subclavian vein. A 39-year-old male volleyball coach presented with progressive swelling of the right arm. Imaging revealed subclavian vein thrombosis and a concomitant pulmonary embolism. No thrombophilia or secondary causes were identified. Given the history of repetitive overhead activity, Paget-Schroetter syndrome was diagnosed. Anticoagulation and activity restriction resulted in symptom resolution within three weeks. This case highlights the importance of recognizing activity-related upper extremity thrombosis in otherwise healthy individuals.
