Pre-exercise meal macronutrient composition and acute inflammatory response in males with type 2 diabetes mellitus

Aims:
Type 2 diabetes mellitus (T2DM) is associated with chronically increased inflammation related to insulin resistance and endothelial dysfunction. Meal macronutrient consumption and physical activity generate acute effects on circulating inflammatory markers in the general population. We aimed to assess the impact of macronutrient composition on the acute inflammatory response to exercise in male adults with T2DM.
Methods:
Ten males with well-controlled T2DM completed five moderate-intensity exercise sessions in a single-blind crossover design under five conditions: 1) fasting and, 2 h after a standardized meal enriched in 2) 22 g sucrose, 3) 22 g fructose, 4) 15 g fat, and 5) aspartame. Blood samples were taken before, during, and after exercise at 15-minute intervals. Levels of C-reactive protein (CRP) were analyzed. Protocol was registered on ClinicalTrials.gov (NCT07175740).
Results:
CRP levels remained unchanged during the exercise and recovery periods in all conditions. CRP levels were higher in the fructose condition compared to sucrose and fasting conditions.
Conclusions:
The inflammatory response to exercise in males with T2DM may be independent of the macronutrient composition of the diet. A meal enriched in fructose could lead to an inflammatory response in these individuals.
