Abstract:
Cleft palatoplasty has evolved substantially in recent decades, with the overarching goal of restoring palatal function for speech while preserving maxillary growth.Despite widespread adoption of advanced techniques such as the Furlow double-opposing Z-plasty, long-term outcomes remain variable. Tissue deficiency, dead space, and postoperative scarring can undermine velar mobility and contribute to persistent dysfunction, ultimately compromising long-term velopharyngeal competence and negating the intended benefits of palatal repair. Additionally, maxillary growth restriction can emerge over time, necessitating maxillary advancement and resulting in velopharyngeal insufficiency (VPI), contributing to the need for secondary speech surgery. In response, innovative, vascularized tissue-augmenting approaches such as buccal fat pad flaps and buccal myomucosal flaps are being increasingly employed to bolster palatal repairs and mitigate factors associated with suboptimal outcomes, as well as secondary palatal lengthening with bilateral buccal myomucosal flaps, for the treatment of VPI has largely replaced pharyngoplasty. This paper examines the pathophysiology of growth-related adverse outcomes following cleft palate repair and presents the rationale, as well as emerging clinical evidence, supporting the use of vascularized tissue-augmenting palatoplasty techniques.
