Immune therapies in type 1 diabetes deserve immune biomarkers

Abstract:
Despite the immune-mediated nature of prevention therapies in type 1 diabetes, current assessment of therapeutic response largely reflects downstream metabolic failure rather than direct immunologic effects. While clinically meaningful, metabolic readouts serve as indirect markers of immune modulation and may therefore provide an incomplete assessment of therapeutic efficacy. The identification and incorporation of biomarkers that enable direct immunologic evaluation would better align outcome measures with the biological pathways these therapies are intended to modify. Such markers may also allow for earlier assessment of treatment response, enabling more timely and informed clinical decision-making.
