Agreement between Mid-pregnancy Serum Placental Growth Factor Levels and Abnormalities in Uterine Doppler Ultrasound

This prospective observational study evaluated the concordance between mid-pregnancy serum placental growth factor (PlGF) levels and abnormal uterine artery Doppler findings in pregnancies at risk of placental insufficiency. Among 127 patients, PlGF testing was discordant with uterine artery Doppler findings in 17% of cases with minimal agreement (κ 0.07; 95% CI -0.13 to 0.28), and only 2 of 10 patients with abnormal uterine artery Doppler had low PlGF. These findings suggest that PlGF and uterine artery Doppler identify different at-risk populations and should not be used interchangeably in placental risk stratification.
