Comprehensive involved-site radiation therapy (ISRT) for oligometastatic disease causes a significant drop in absolute lymphocyte count (ALC). However, treatment-related lymphopenia did not independently impact overall survival in this patient group.

Main Results:

  • Median ALC decreased significantly from baseline (1400 cells/µL) during and after ISRT.
  • While 27% developed grade ≥3 lymphopenia, it was not independently associated with OS on multivariable analysis.
  • ECOG performance status, age, albumin, and pre-radiation chemotherapy were independent predictors of OS.

Conclusions:

  • ISRT for oligometastatic disease leads to a sustained reduction in ALC.
  • Treatment-related lymphopenia was not an independent prognostic factor for OS in this cohort.
  • The benefits of metastasis-directed therapy may exceed potential immunologic effects of radiation-induced lymphopenia.
