Intralesional 5-aminolevulinic acid photodynamic therapy (ALA-PDT) effectively treats deep extramammary Paget's disease (EMPD) when surgery is not feasible. This minimally invasive approach overcomes topical PDT limitations, achieving complete regression with good tolerance.
