Abstract:
Spontaneous bilateral renal subcapsular hematoma (SRSH) is a rare condition, especially in young individuals. We report a 25-year-old Asian female presenting with acute bilateral flank pain and new-onset hypertension. Contrast-enhanced CT revealed bilateral SRSH without an identifiable cause. The hypertension, consistent with the Page kidney phenomenon, resolved within five days under conservative management. Follow-up imaging confirmed complete hematoma resolution. This case highlights SRSH as an important differential diagnosis for acute flank pain and demonstrates that Page kidney-related hypertension can be reversible, supporting conservative management in hemodynamically stable patients after exclusion of malignancy.
