Abstract:
In patients with congenital cervical stenosis, the addition of abnormal cervical facet orientation (horizontal or hypoplastic joints) may predispose to segmental instability, subluxation, and canal compromise. Full assessment of facet anatomy is essential pre-operatively, as selected patients (e.g., those with demonstrable subluxation, widened facet joint spaces, or high risk of adjacent-segment instability) may require instrumented fusion in addition to decompression to maintain stability.
