Evaluation of routine four-quadrant re-excision in patients undergoing breast-conserving surgery

Objective:
The aim of this study was to evaluate the contribution of routine four-quadrant re-excision performed during breast-conserving surgery in detecting de novo tumor foci and ensuring negative surgical margins in patients with breast cancer.
Methods:
A retrospective analysis was conducted on 44 female patients who underwent breast-conserving surgery between April 2021 and April 2023. In all cases, routine re-excision was performed in the superior, inferior, medial, and lateral quadrants during the same operative session. Based on pathological findings, patients with additional tumor foci in the re-excision specimens were classified as Group 1, while those without were classified as Group 2.
Results:
The mean age of the patients was 54.2±11.6 years, with an average follow-up period of 21.5±8.3 months. Additional tumor foci were identified in seven patients (15.9%). There were no statistically significant differences between the groups in terms of age, tumor size, hormone receptor status, or histopathological characteristics (p>0.05).
Conclusion:
Routine four-quadrant re-excision may aid in the identification of microscopic de novo tumor foci even when clear surgical margins are achieved. However, larger multicenter studies with longer follow-up periods are warranted to determine its potential impact on local recurrence and long-term outcomes.
