Summary

Parkinson's disease disrupts dopamine release patterns in the basal ganglia, affecting motor control and multitasking. Early PD patients show altered dopamine distribution, losing the distinct somatotopy and functional segregation seen in healthy individuals.

Background:

The basal ganglia possess somatotopic organization within parallel, segregated corticostriatal circuits.
The impact of Parkinson's disease (PD) on this architecture remains unclear.

Purpose of the Study:

To investigate task-evoked dopamine release in the basal ganglia of early Parkinson's disease (PD) patients compared to healthy controls (HC).
To examine the effects of PD on the somatotopic and functional segregation of corticostriatal circuits.

Main Methods:

Utilized [11C]raclopride positron emission tomography/magnetic resonance imaging (PET/MRI) to map dopamine release.
Assessed 13 early PD patients and 15 HC subjects during motor, cognitive, and reward tasks.

Main Results:

In PD, motor tasks showed reduced dopamine release in the dorsal putamen but increased release in the caudate and ventral striatum compared to HC.
Healthy controls displayed distinct upper vs. lower limb somatotopy in dopamine release, which was absent in PD.
HC exhibited segregated dopamine release patterns across tasks; PD patients lost both anatomical and functional segregation.

Conclusions:

Altered dopamine release topography in PD may offer compensatory benefits for maintaining function.
The loss of segregation in PD could contribute to difficulties in motor selection and multitasking.
