Atrial Septal Erosion: Very Rare Complication of Percutaneous PFO Closure

Background:
Percutaneous patent foramen ovale (PFO) closure is an established method for secondary prevention of paradoxical embolism in patients with cryptogenic stroke. Residual right-to-left shunts after PFO closure are observed in up to 25% of patients.
Case Summary:
We present the clinical case of a 56-year-old man with cryptogenic stroke who underwent transcatheter PFO closure. Six months postprocedure, echocardiographic follow-up revealed a progressive residual right-to-left shunt due to marginal septal erosion. A first attempt at transcatheter closure of the residual defect was unsuccessful. Subsequently, a trans-septal puncture with deployment of an overlapping PFO occluder was performed, resulting in complete closure of the residual shunt.
Discussion:
This case highlights the importance of systematic echocardiographic surveillance and individualized intervention strategies in managing rare device-related complications after PFO closure.
