A 31-year-old woman presented as a level 1 trauma activation after sustaining multiple stab wounds to the left chest. Computed tomography identified pneumopericardium and a left ventricular hypodensity concerning for thrombus or laceration. Transthoracic echocardiography revealed a mitral valve mass. Transesophageal echocardiography further characterized it as a 2.0 × 1.5 cm cystic structure attached to the ventricular aspect of A2/A3 scallops of the mitral leaflet. Cardiac magnetic resonance confirmed a water-based cystic lesion with no perfusion or enhancement, establishing the diagnosis of a congenital blood cyst.
