Abstract:
Combined liver trauma and renal artery thrombosis are severe and rare injuries that may lead to hemorrhagic shock and extensive renal infarction if timely intervention is not performed. We report the case of a 15-year-old male with Grade IV liver trauma (AAST 2018 classification) associated with complete thrombosis of the right renal artery. The patient underwent emergency hepatic arterial embolization for hemostasis, followed by renal artery thrombectomy and balloon angioplasty, resulting in reperfusion of most of the renal parenchyma. During follow-up, a pseudoaneurysm and dissection of the right renal artery were detected and successfully treated with covered stent placement, completely excluding the lesion and restoring arterial flow. The patient recovered well and was discharged. This case highlights the crucial role of multiphasic computed tomography (CT) in identifying combined hepatorenal injuries and confirms the effectiveness of endovascular intervention in controlling hemorrhage, restoring renal arterial patency, and managing complications, thereby maximizing organ function preservation in young patients.
