Buccal mucosal graft interposition for recurrent ileal conduit stomal stenosis: A case report

Recurrent ileal conduit stomal stenosis is challenging, particularly in patients with prior abdominal surgery and poor local tissue quality. We present a 43-year-old female with recurrent stenosis after ileal conduit urinary diversion and prior stomal revision. A 4 × 5 cm buccal mucosal graft was applied as a segmented onlay at the mucocutaneous junction following circumferential stomal mobilization and adipocutaneous flap advancement. Despite partial wound dehiscence, the graft remained viable postoperatively. At 6-month follow-up, the stoma was healthy, well-everted, and accommodated a 14 Fr catheter. This technique may offer an alternative for select patients with recurrent ileal conduit stomal stenosis.
