Abstract:
Emergency-only hemodialysis (EoHD) for undocumented immigrants with end-stage kidney disease (ESKD) in the United States represents a striking contradiction in contemporary health policy. Although ESKD is a chronic and fatal condition that requires regular dialysis to sustain life, many undocumented patients are only granted access to treatment when they deteriorate into a medical emergency. This conceptual commentary examines EoHD as a political and moral arrangement at the intersection of immigration control and public health. The analysis combines Michel Foucault's concepts of biopolitics and governmentality, Achille Mbembe's necropolitics, and Paul Farmer's notion of structural violence. It also engages comparatively with Adriana Petryna's and Julie Livingston's ethnographic works to interpret the impact of bureaucratic thresholds, exclusion, and repeated cycles of crisis on the lived experience of undocumented patients, their families, and the clinicians who care for them. The commentary argues that EoHD functions as a biopolitical mechanism through which the state regulates access to life-sustaining care based on legal status, and concludes that EoHD is a form of structural harm and an indirect tool of migration governance, with consequences that extend far beyond the clinical setting.
